I would like to extend my sincerest apologies for the inconvenience caused by my inability to provide the required documents, namely the Baptismal certificate and Form 137. Unfortunately, I have encountered difficulties in obtaining these documents due to the considerable distance between my current location in Luzon and my birthplace in Ezperanza, Agusan Del Sur, Mindanao. Moreover, I have lost contact with my relatives, some of whom no longer reside in the area.

After completing my high school education, I relocated to Manila, the capital city of the Philippines, to pursue a college degree. Regrettably, the local schools I attended during my elementary and high school years, aside how rural my place is, have encountered numerous challenges, including severe typhoons, which have resulted in the loss of records from the year 2000.

Regrettably, I currently find myself in a situation where distance and technological constraints are impeding my ability to acquire and transmit these documents effectively. I deeply understand the importance of submitting complete and accurate documentation, and I sincerely apologize for any inconvenience or delay this may have caused.
